The information below is for those that have already been accepted and paid their deposit to the Marriage & Family Therapy program at Friends University. Enrollment for MFT students begins in July of each year.

MSFT Enrollment Documents:
- There are two documents that need to be filled out, signed and returned:
- Continuous Registration Agreement Course Outline
- SPA MSFT form
- The FERPA form will need to be completed online following the instructions on the document.
- **Please note: The forms below will be updated in July and the Recruiter/Student Services Coordinator will instruct you when to complete them.
Wichita:

Kansas City:

Please sign and return the Continuous Registration Agreement Course Outline and SPA MSFT Forms for your appropriate program location via email, fax, or mail:

Reminder – Complete TB Questionnaire: (if you have not done so already)
You must complete a required Tuberculosis (TB) questionnaire in order to be able to enroll for the MSFT program.
Online Instructions:
- Go to www.friends.edu
- Hover over Students
- Click on Self-Service Banner
- Click on Student Self-Service Banner
- Login using your Friends University network username and new password
- Click on TB (Tuberculosis) Questionnaire
- Complete the short survey
- Click Submit
